Transition and Laterlife Matter is a well-established organisation, through the delivery of services, including our Supported Living Services, we have built up strong working relationships with partners and local authorities. We work closely with our local councils and those of neighbouring boroughs to assess referrals for individuals in need of supported living accommodation and support.

Service Model
We know that finding the right support to meet your needs can be difficult.
Here at Transition and Laterlife Matter we provide supported living accommodation services that are designed around what’s important to you, for you to live a full life and maintain independence.
Supported Living offers you an alternative to residential care, with a combination of housing and support services.
Supported Living will empower you to design your support, choosing the right type, amount, and time you need support. This enables you to live an independent, productive and fulfilling life that is meaningful to you.
Supported Living can be coupled with other services, such as job training, life skills, education. We support individuals with a range of needs such as those diagnosed with mental health conditions, autism, acquired brain injury, or learning disability.
The support you receive is controlled by you, you will have a personalised support plan based on your interests, likes, and skills. This may include help with personal care, managing finances, finding work or leisure activities, and building new friendships. It’s up to you!
Our supported living services focus on developing environments that focus on promoting and encouraging the people we support’s independence and skills.
Support according to different levels of need
The support we offer will depend on your level of need. We ensure highly skilled staff are available within your service depending on your needs and requirements. One to one support is provided to you as required by your assessment and personalised support plan.
Shared accommodation may have staff on-site for between 4-12 hours per day, with access to an on-call manager to support you when staff are not on site.
Higher needs accommodation will have 24-hour care and support and a sleep in or waking night support worker.
Referral & Assessment
All referrals will be fully assessed, to find out your needs, aspirations and the accommodation that is the most effective solutions for you.
Placement is only ever offered following a full assessment.
The assessment takes place with your full involvement, as well as people who are important to you and any other professionals involved. You are invited to view suitable placements before making a choice.
Costs of the placement will be agreed with your placing authority as part of the planning process.
If you are offered accommodation with us
You will be given the opportunity to look around the property again and meet anyone else living in the property. You may want to visit a few times to decide if the property is right for you. You will be able to meet support workers and chat with them about the area and what there is to do in the community that you may like.
Your tenancy agreement will be explained to you, and we may go through a “pictorial” tenancy agreement with you to explain your rights and responsibilities as a tenant.
We will explain different parts of your tenancy, such as moving in, managing your rent payments, reporting repairs, getting along with your neighbours and how to give us your feedback.
You may have an ‘appointee’ who has been appointed by the Court of Protection to sign your tenancy agreement on your behalf. If so, we will also go through the agreement with them.
Our support workers are friendly and helpful and will ensure you have everything you need to be happy in your new home.
